·         Hereby give permission for my son / daughter / ward to attend the St Anne’s youth group outing and acknowledge that the organisers, whilst exercising all reasonable care, will not be held liable for any injuries or damages. The youth group will be ten-pin bowling at the Annesville Bowling Alley.
